While applying for a Home Care Package may seem complex, the process is essentially just 5 straightforward steps that Ametz Community Care can help guide you through.
Step 1 — Confirm your eligibility. You'll generally need to be aged 65 or over, live in Australia, want to continue living in your own home, and require some support to maintain your independence.
Step 2 — Register with My Aged Care. Contact My Aged Care to register and schedule a home visit with ACAT (Aged Care Assessment Team). Our team can assist you with this initial contact and help prepare for your assessment.
Step 3 — Complete your ACAT assessment. The ACAT team will visit your home to evaluate your eligibility and determine the appropriate level of care you require. My Aged Care will follow up with a letter within a few weeks, informing you whether you've been approved for a package.
Step 4 — Join the National Queue. Once approved, you'll be placed on a waitlist called the National Queue. Depending on your approved Home Care Package level, your wait time can range from a few weeks to several months.
Step 5 — Receive your package assignment. You'll be notified by letter when a Home Care Package is assigned to you. You then have 56 days to choose your preferred provider like Ametz Community Care and activate your package to begin receiving the support you need.
