Ametz Community Care - NDIS

Our Mission

Ametz Community Care was established with a fundamental and transformative mission: to deliver NDIS services that empower you to live life according to your own vision and values.

As a personalized care provider, you will always connect with genuine team members who are passionately committed to enhancing your everyday experiences and quality of life. We invest time in truly understanding who you are – your passions and preferences, your aspirations and requirements – and work diligently to pair you with a support worker who resonates with your cultural background, personal interests, and lifestyle goals.

NDIS Support Coordination

Quality support coordination can transform your experience with The National Disability Insurance Scheme (NDIS). At Ametz Community Care, we're dedicated to delivering exceptional support services that help you achieve your personal goals and experience the most fulfilling life possible.

What is NDIS support coordination?

The NDIS represents a groundbreaking federal government initiative providing disability support across Australia. While this comprehensive scheme offers tremendous opportunities, many participants find the system challenging and overwhelming to navigate independently.

Recognizing these complexities, the NDIS provides support coordination services. The government describes this as "a capacity building support to implement all supports in a participant's plan, including informal, mainstream, community and funded supports."

This means professional assistance is available to help you navigate the NDIS system effectively, ensuring you maximize the value of your plan and access the disability support services you need and deserve.

What is the role of a Support Coordinator?

A Support Coordinator serves as your guide and advocate, helping you understand and effectively implement the funded supports within your NDIS plan while maintaining connections to community, mainstream, and other government services.

Specifically, your Support Coordinator will assist you in negotiating with disability service providers regarding their offerings and associated costs within your NDIS budget. They ensure you locate appropriate support services regardless of complexity. Your coordinator also manages service agreements and bookings while building your capacity to exercise greater choice and control over your plan implementation.

Essentially, a Support Coordinator works to optimize your plan, ensuring you receive maximum benefit from your federally funded supports.

Where do I get a Support Coordinator from?

You can locate registered support coordination providers through the Provider Finder tool in your NDIS participant portal myplace. This tool helps identify support coordination services in your local area. The NDIA publishes updated lists of registered providers by state quarterly on the NDIS website.

If you or someone you care about needs NDIS support coordination services, contact Ametz Community Care to explore whether one of our experienced Support Coordinators aligns with your specific needs and circumstances.

Our Support Coordinators also assist with NDIS review preparation and advocate on your behalf when necessary, ensuring you maintain access to the support level required for a fulfilling, independent life.

Who can get support coordination funding?

Not all NDIS participants automatically receive support coordination funding. However, every NDIS participant has the right to request this service. NDIS participants seeking support coordination should raise this topic during their NDIS planning meetings. The NDIS evaluates each participant's needs individually when considering funding requests.

What Are the 3 Levels of Support Coordination?

Support Connection

This entry-level support coordination option is designed for participants with less complex needs who can manage many supports independently. Support Connection focuses on connecting participants with informal, community, and mainstream support services.

Support Coordination

This intermediate level provides more comprehensive assistance and coordination than Support Connection. It suits participants with moderate complexity needs. Support coordination helps participants navigate and implement their NDIS plans, manage service delivery responsibilities, and ensure all supports work effectively together.

Specialist Support Coordination

Specialist support coordination represents the highest level of support coordination services. This level serves participants with complex needs requiring specialized expertise. Specialist Support Coordination involves advanced professional knowledge and is typically delivered by professionals with backgrounds in health, social work, or complex support management.

The goal of specialist support coordinators is ensuring services and supports are not only connected but fully integrated to achieve consistent service delivery and optimal participant outcomes.

How we can help with support coordination

NDIS 5 Step Application Guide

Navigate your NDIS application journey with Ametz Community Care's comprehensive 5-step guide, designed to help you successfully apply for the support you need and deserve.

Before You Begin:

Review our NDIS eligibility requirements checklist:

✓ Are you aged between 7 and 65 years?

✓ Do you hold Australian residency and live in Australia?

✓ Do you require regular personal assistance due to a permanent and significant disability?

✓ Do you rely on specialized equipment as a result of a permanent and significant disability?

✓ Would early intervention supports help minimize your future support requirements?

Step 1: Submitting Your Application

Individuals who believe they meet NDIS eligibility criteria can obtain an Access Request Form (ARF) and Supporting Evidence Form (SEF). Work collaboratively with your GP, allied health professionals, and medical specialists to gather and prepare all required documentation.

Eligible individuals can apply through several methods:

  • Call 1800 800 110 to submit a verbal access request directly, or
  • Download and complete the ARF digitally, then email it to the NDIA with all supporting documentation, or
  • Download the ARF, print the completed form, and post it to the NDIA with supporting materials, or
  • Email NAT@ndis.gov.au requesting an ARF be mailed to your address, or
  • Contact your regional NDIA office directly and request an ARF be posted to you.

Step 2: Understanding Your Access Decision

Once you've submitted all required information, the NDIA will evaluate your eligibility for NDIS services and supports.

You'll receive written notification regarding their decision through an official 'access decision' letter.

If Your Application Is Approved

When deemed eligible for NDIS support, you'll be contacted to schedule a planning meeting where you'll discuss your specific support requirements and funding needs.

If Your Application Is Declined

Should you not meet NDIS eligibility criteria, you can still receive valuable information and assistance from an NDIS Local Area Coordinator (LAC) who can connect you with community-based supports and services.

If you disagree with the NDIA's access decision, you have the right to request a review through the Administrative Appeals Tribunal (AAT).

Visit the AAT website at www.aat.gov.au or contact them at 1800 228 333.

Step 3: Planning Meeting Preparation

The NDIS provides a comprehensive preparation checklist to help you get ready for your planning meeting, which you can access through their official resources.

The NDIA strongly recommends completing Booklet 2 - Planning. You can work through this independently or seek assistance, then bring your completed booklet to your planning meeting.

Remember to gather and bring all relevant medical reports, assessments, and professional evaluations. These documents help the NDIS understand your specific support requirements.

Based on your individual circumstances and support needs, various professionals can assist you throughout the planning process. Research who can best help you develop your plan.

Step 4: Your Planning Meeting Experience

Participating in your planning meeting—whether conducted in person or via telephone—is essential for the NDIS to collect comprehensive information about your needs, enabling collaborative development of your optimal support plan. During this important meeting, you'll discuss your personal goals, daily activities, and objectives you wish to achieve.

Your planning meeting also provides an opportunity to request Support Coordination funding. Support Coordinators help you develop the skills necessary to understand, implement, and effectively utilize your NDIS plan. Ametz Community Care employs experienced and dedicated Support Coordinators who can assist you if Support Coordination funding becomes part of your approved NDIS Plan.

Step 5: Receiving Your Approved NDIS Plan

Following plan approval, you'll receive your NDIS plan through multiple channels: delivered in person, mailed to your address, and accessible through the myplace participant portal.

During your planning conversation, you can request your plan be provided in various accessible formats including Braille, electronic text (CD format), large print, audio (CD format), or translated into your preferred language.

The NDIS provides detailed information about plan implementation and support access through their official resources. For additional assistance, contact the NDIS directly at 1800 800 110.
